You have a choice of ways that you can register as a patient.
Download and complete a GMS1 form from this page. To help us understand your medical history it would be helpful if you would also download and complete a new patient questionnaire – see link below. Both these forms should be handed in at our reception desk.
The GMS 1 new patient registration form and the new patient questionnaire forms are also available at our reception. You can collect them there for completion at your leisure – or you can fill them in at our reception
Please bring some form of identification – preferably a utility bill, driving licence or passport.
Please see the Charter page to read about your rights and responsibilities as a patient. We advise that you check the information on this website about the practice and the vast number of services available nationally and locally which form part of your NHS. Many other support organisations exist in the voluntary sector.

TO JOIN THE PRACTICE
Please complete a GMS1 form and a New Patient Questionnaire for each person. You can download/print the forms below or ask at reception.

PRACTICE AREA
Our practice inner boundary area includes the City of Hull to the east of the River Hull and to the boundary on the eastern side but excluding North Bransholme. This includes all or part of the following postcodes: HU7, HU8, HU9 and HU11. Our outer boundary includes patients in Bilton, Coniston, Thirtleby, Ganstead, Swine, and Wyton – with postcode HU11.
NEW PATIENTS
We are a group practice and you may see any doctor by choice – subject to their availability. During busy periods, or if the doctor is away, please be prepared to see one of the other GPs.
