⃰ A non-refundable deposit of £15.00 is required when requesting any form or letter and the balance will be required at the time of collection.
Please allow a minimum of 25 working days for any request to be completed.
We will inform you when the form / letter is ready for collection.
We will hold the letter / form at the Practice for a further 25 working days and if are not collected within that time period, they will be destroyed.
If your request is not on the above list: a fee will be provided prior to the request being actioned.
